Case Manager - Refugee Program
Southern Jersey Family Medical Centers, Inc. is looking for talented a Case Manager to step into our culturally diverse and fast-paced facility and work with our Refugee Program
Summary: The Case Manager of the Refugee Program functions as the liaison between SJFMC, the State Refugee Health Program and resettlement agencies. The ideal candidate will coordinate health screening services, document, and provide follow up to routine and requested reports.
Hours: This is a full-time position that requires the ability to work a flexible schedule. General daytime hours Monday – Friday with rotating Saturdays and at least one late night per week.
